Supplies Required for Genetic Sample Collection

1. One 44 ml Scope^® Cool Mint mouthwash.
2. One 50 ml sample free-standing collection container.

Protocol for Collection of Genetic Sample

1. Ask patient to take about 10-15 ml of mouthwash (1/3 of container) into mouth and swish vigorously for about one  minute.

2. Have patient spit mouthwash into the 50 ml sample collection container.

3. If tolerated, ask patient to repeat steps 1 & 2 two more times, always spitting into the same 50 ml sample  collection container.

4. Complete the sample collection label with the following:

a. Patient identifier
b. Patient initials
c. INVEST Site number
d. Date

6. Discard the empty Scope^® bottle.

7. Sample may be stored at room temperature or in refrigerator until shipped.

8. Samples may be stored up to 3 weeks before shipping.

Shipping Genetic Samples

1. Use the enclosed shipping label to ship all samples by overnight express.  Insure that the sample container is labeled and the cap is well secured to avoid leakage of samples.

2. It is not necessary to place samples on dry ice before shipping.

3. Do not ship samples on Fridays, Saturdays, or the day before a holiday.

4. Notify the core laboratory by email at kathy@cop.ufl.edu when a shipment is made.

5. Include a sample shipment summary sheet in each shipment.
