Moonlighting Event Requests
Please print out the appropriate form for the event you are requesting permission to do. Completion of the correct form, including signatures, is required prior to authorization. Forms are due in the housestaff office by Noon on Friday prior to the weekend moonlighting event requested.
Any moonlighting activities not listed on this page are not approved and your participation in them will result in disciplinary action, not excluding, termination.
